How We Help Patients & Families

LifeTouch Health enables patients with a projected lifespan of six months or less, due to serious illness, to live their lives to the fullest in their homes, on their terms. Patients are cared for in their own or a family member’s home, a nursing home, a hospital, an assisted living facility or at one of our LifeTouch Health inpatient or contracted facilities across our 44-county service area. Physical, emotional and spiritual support is provided based on the patient’s wishes and the family’s unique needs.

  • Admissions 24 hours a day, 7 days a week

  • Team on call 24 hours a day, 7 days a week

  • Decreases physician office calls, 911 calls, hospitalizations

  • Offers psychosocial support and medication intervention, including support for anxiety and depression

  • Increases caregiver and patient support

  • Assists with end-of-life planning (prepares funeral arrangements, obtains DNR order)

  • Assists with ADLs

  • Regulates medications

  • Covers medications, equipment and supplies related to the terminal diagnosis

  • Monitors pain with every visit
